Job summary

Sanctuary Care is seeking an Advanced Nurse Practitioner to join our regional team covering Home Counties and West London. The role offers £65,000 per year plus a car allowance of £9,800.73, for a 35-hour week.

Responsibilities

Support the Head of Clinical and Advanced Practice in delivering advanced practice across Sanctuary Care homes.
Provide autonomous clinical advice, leadership and education to nursing and residential teams.
Deliver expert care through clinical assessment, diagnosis, treatment and evaluation with compassion and professionalism.
Collaborate with GPs, community services, hospitals and internal teams to ensure residents receive the right care at the right time.
Lead frailty-focused triage and assessment via phone, video and in-person to avoid unnecessary hospital admissions.
Mentor nurses undertaking advanced training and contribute to improvement projects and research.
Develop and maintain clinical policies, procedures and competency frameworks.
Drive measurable improvements in resident outcomes through innovation and clinical excellence.
