Achieve Together is seeking a Referrals Coordinator to join the community hub team in Leatherhead. You will manage referrals from enquiry to admission, ensuring smooth coordination and timely delivery of services within capacity and governance requirements.

The role requires strong organisational skills, attention to detail, and experience in administrative or coordination roles. You will support Growth and Partnerships Managers and use Excel to track progress, contributing to excellent customer

Qualifications

  • Previous experience in an administrative, coordination, customer service or support role.
  • Strong organisational skills with the ability to manage multiple referrals, competing priorities and time-sensitive actions.
  • Good attention to detail with accurate record keeping and following processes.
  • Confident user of Microsoft Excel for trackers, updating spreadsheets and basic reporting.

Responsibilities

  • Manage the referral journey from initial enquiry through to admission, ensuring each referral is coordinated smoothly, accurately and within agreed timeframes.
  • Keep referral information, funding details, placement paperwork and admission administration up to date and clearly tracked.
  • Provide administrative and coordination support to Growth and Partnerships Managers and operational teams.

We are seeking a highly skilled and motivated Referrals Coordinator to join our community hub team. The successful candidate will play a crucial role in supporting the organisation's core operations and ensuring the efficient and effective delivery of services.


Key Responsibilities

The Referral Coordinator will manage the referral journey from initial enquiry through to admission, ensuring each referral is coordinated smoothly, accurately and within agreed timeframes. The role is responsible for keeping referral information, documentation, funding details, placement paperwork and admission administration complete, up to date and clearly tracked. Reporting to the Commercial Manager, the Referral Coordinator will provide administrative and coordination support to Growth and Partnerships Managers, operational teams and the wider Community Hub. They will help ensure referrals progress in line with home capacity, placement suitability, local priorities and commercial governance requirements. This role requires strong organisation, attention to detail, system accuracy and clear communication, with a focus on delivering a consistent, timely and positive customer experience throughout the referral pathway.


Requirements


  • Previous experience in an administrative, coordination, customer service or support role, ideally involving accurate record keeping, diary coordination, data entry or process tracking.

  • Strong organisational skills, with the ability to manage multiple referrals, competing priorities and time-sensitive actions in a structured and consistent way.

  • Good attention to detail, with the ability to maintain accurate records, update systems, check documentation and follow agreed processes.

  • Confident user of Microsoft Excel, with the ability to maintain trackers, update spreadsheets, organise data and support basic reporting.
