The Forward Trust is seeking a dedicated Substance Misuse Recovery Worker for our Dover service. You will deliver person-centred support to adults with drug and/or alcohol challenges, using both harm reduction and abstinence-based approaches as needed.

You will manage your own caseload, carry out comprehensive assessments, and develop SMART care plans. The role involves one-to-one work, group sessions, and collaboration with housing, health and community services to promote sustainable recovery.

Formación

• • Level 3 Diploma in Health and Social Care or equivalent.
• • Understanding of issues faced by people experiencing drug and alcohol misuse.
• • Knowledge of Recovery Agenda and different approaches to supporting recovery.
• • Understanding safeguarding responsibilities and procedures.
• • Experience working within substance misuse services.
• • Experience undertaking comprehensive assessments, risk management and SMART care plans.
• • Experience delivering structured interventions and facilitating group-work.
• • Ability to use motivational interviewing effectively in one-to-one and group settings.
• • Strong IT skills and maintaining accurate, timely records.
• • Understanding of continuity of care and wider community services.
• • Commitment to equality, diversity and inclusion.

Responsabilidades

• Deliver psychosocial interventions within a substance misuse framework, using harm-reduction and abstinence-based approaches as appropriate.
• Manage an individual caseload with comprehensive assessments, risk identification and personalised SMART care plans.
• Deliver one-to-one interventions and facilitate group sessions, including structured treatment programmes.
• Build effective relationships with housing, healthcare, probation and other services to provide holistic support.
• Contribute to service targets, maintain accurate records and ensure care aligns with local commissioning expectations.
• Flexibility for occasional evening/weekend work and travel to other projects as needed.
