Job summary

South West London & St George's Mental Health NHS Trust is seeking a Recovery Support Worker to join the Carshalton & Wallington Integrated Recovery Hub. You will work with service users in a community setting, delivering recovery-oriented interventions and intensive engagement to those in distress.

You will collaborate with a multidisciplinary team, carers and local partners to develop and implement care plans, monitor progress, and promote healthy lifestyles.

Responsibilities

• Provide recovery-focused nursing care through therapeutic engagement.
• Undertake 1-1 engagement with service users in an honest, open and non-judgemental way.
• Collaborate with the MDT and external partners to develop care plans.
• Monitor and record clinical observations and vital signs; escalate deterioration.
• Promote healthy lifestyles and deliver health promotion advice and support.
• Escalate concerns to the appropriate Health Care Professional.

Job description

1. Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support.
2. Supporting the patients management of care needs through:
   1. Initial data collection
   2. Monitoring of individual service user / patient progress
   3. Feedback and discussion
   4. Producing reports and in-care reviews
   5. Devising a plan of care and interventions supervised by a Registered Nurse.
3. In a junior leadership role leading on and directing junior to staff in providing hands-on care to service users including:
   • Undertaking 1-1 engagement with service users in an honest, open and non-judgemental way.
   • Recognising and valuing service users as individuals, acknowledging the importance of always maintaining the service user's respect and dignity and ensure that the care provided respects equality and diversity.
   • Contributing to the development of care plans that reflect service user needs and relevant patient care documentation working in partnership with service users, wider MDT and friends/relatives/significant others.
   • Escalating any concerns to relevant Health Care Professional.
   • Monitoring and recording service user clinical observations, such as physical observations and vital signs and escalating as appropriate any signs or symptoms of deterioration or that do not fulfil the personal parameters to the relevant professional (Registered Nurse or Doctors). Promote healthy lifestyles and give health promotion advice and support.
   • Implementing the principles of "recovery": encouraging and supporting service users to engage with healthy lifestyles, facilitating psychosocial activities and other activities of their interest. Where appropriate contributing to psychological interventions under the guidance of a registered professional.
4. Escort service users, as required, in line with Trust policies.
5. Advocating for service users' needs and rights within Trust policy. Encouraging and empowering service users to have an optimum level of responsibility for their individual programme of care.
6. Supporting the registered nurse to coordinate and prioritise activities within the clinical area.

The successful candidate will be able to offer intensive engagement work and recovery-orientated interventions to people using services, particularly those in a heightened state of distress, and who may require additional levels support.

You must be able to develop positive relationships with clients, adopting a pragmatic and non-judgemental approach, encourage and support individuals to take part in activities suited to their particular needs and interests, in a way that helps progress individual recovery plans. You will be able to support and enhance the therapeutic care of individuals within the community, including contributing to meaningful care plans, risk management plans, and delivering appropriate and therapeutic interventions.

You will work collaboratively with individuals in other community involvements -e.g. Supporting patients with housing, employment, education, welfare benefits, social and peer networks. You should be able to demonstrate excellent communication and interpersonal skills and an ability to work collaboratively with individuals adopting a creative / problem solving approach to working practice.

You should be committed to Multi-Disciplinary Team working and partnership working, establishing good networks with colleagues in the Home Treatment Teams, Recovery and Support Teams, In-Patient services, Police, GPs and a variety of voluntary, statutory and independent agencies.

The Carshalton & Wallington Integrated Recovery Hub is a community-based mental health service for working-age adults between 18 to 70 years, who have experienced severe and enduring mental illness. The Carshalton & Wallington IRH works collaboratively with clients, their carers and networks with the aim of minimising the impact of mental disorder has on their lives. The friendly team is made of integrated health and social care professionals. We work in close partnership with GPs, Housing, Police and other key partners, including voluntary organisations.

Carshalton & Wallington IRH is a stable and cohesive team built on mutual trust, collaboration, teamwork and support. This largely explains our stability and very little or no turnover.
