Job summary

The Forward Trust is seeking a Recovery Worker in Ashford to join our team on a full-time basis (35 hours per week). You will support adults 18+ with drug/alcohol challenges as part of an integrated healthcare team, delivering person-centred support and helping clients progress along their recovery journeys.

Key duties include caseload management, assessments, care planning, harm reduction, and coordinating access to housing, benefits, employment and healthcare services.

Responsibilities

• Manage your own caseload and support clients through recovery journey.
• Complete assessments, risk assessments and care plans.
• Deliver structured 1:1 interventions and group sessions.
• Provide harm reduction advice and practical support.
• Help clients access housing, benefits, employment, training, healthcare and community services.
• Work closely with healthcare professionals, probation and partner agencies.
• Review care plans and support positive outcomes.
• Maintain accurate records and work towards service targets and KPIs.
