Job summary

South London and Maudsley NHS Foundation Trust is seeking an experienced Recovery Coordinator to join a specialist drug and alcohol service. You will support service users throughout their treatment and recovery journey as part of a multi-disciplinary team.

We are looking for strong experience in substance misuse, recovery coordination, care planning and multi-agency working, with a passion for helping individuals achieve positive and sustainable change.

Qualifications

  • Experience in screening, assessment, recovery planning and risk management.
  • Strong communication, interpersonal and organisational skills.
  • Ability to work collaboratively with service users and external agencies.
  • Commitment to Equality, Diversity and Inclusion.
  • Caseload management and prioritisation.

Responsibilities

  • Act as a main care coordinator for an assigned caseload of service users.
  • Complete screening, comprehensive assessments, asset mapping and recovery planning.
  • Develop, review and coordinate personalised recovery plans.
  • Provide 1-to-1 key work and group work where required.
  • Coordinate access to psychosocial, clinical and harm reduction interventions.
  • Carry out risk assessments and develop risk management plans.
  • Triage service users and refer to relevant treatment and community services.
  • Work closely with prescribing clinicians to support outcomes.
  • Promote recovery, peer support and social reintegration.
  • Support access to mutual aid and community resources.
  • Maintain accurate case records and contribute to service data.
  • Support the smooth running of the service and duty cover when needed.
  • Promote inclusive engagement with hard-to-reach communities.
