Hertfordshire Partnership University NHS Foundation Trust in Hemel Hempstead is seeking a Support Worker to join a team delivering mental health services to service users and carers. The role focuses on accommodation solutions and recovery principles within the Care Programme Approach.

You will work across boundaries, support the Team Leader, and help promote independent living and community integration, with commitments to personal development and team-based care.

Competenze

• Commitment to Continuing Personal Development.
• Three years experience of providing mental health services from within a team setting.
• Car driver unless you have a disability as defined by the Equality Act 2010.

Mansioni

• Work as part of a team providing mental health services to service users and their carers.
• Focus on accommodation solutions using the Support Time and Recovery Model.
• Work across service and/or care group boundaries as necessary.
• Collaborate with the Team Leader and other staff to maximise STAR services.
• Contribute to Care Programme Approach / Care Management processes with a Recovery focus.
• Provide support to promote recovery and independence, and to support an allocated group of service users and carers.
• Contribute to management tasks within a STAR team and support the Care Coordinator in implementing recovery-focused care plans.
• Ensure service users engage with the Care Plan and access services regularly and consistently.
