Job summary

Somerset NHS Foundation Trust is seeking a compassionate Support Time and Recovery Worker for the Assertive Outreach Team in Bridgwater. This role supports adults with severe mental health challenges to recover and live independently in the community, inside a multidisciplinary team.

You will deliver evidence-based interventions, provide practical and emotional support, and help service users engage with care plans while maintaining professional boundaries and ensuring appropriate discharge

Qualifications

• NVQ Health and Social Care or equivalent, or willingness to undertake training.
• Experience caring for people and working in community settings.
• Ability to engage with care plans and recovery goals.

Responsibilities

• Support individuals with mental health difficulties to engage with care plans and recovery goals.
• Deliver evidence-based interventions under supervision, including behavioural activation and emotional regulation.
• Provide practical, emotional and social support in community and discharge planning.
• Build therapeutic relationships while maintaining professional boundaries.
• Promote independence, dignity, wellbeing and social inclusion.
• Assist access to accommodation, benefits, advocacy and community resources.
• Monitor progress, spot early relapse signs and report concerns to the clinical team.
• Collaborate with carers, families and partner agencies for coordinated care.
• Maintain confidential clinical records in line with Trust policies.
• Participate in team meetings, supervision and quality improvement activities.
