Job summary

The Forward Trust is recruiting a Health and Wellbeing Practitioner to join the Substance Misuse Service at HMP Brixton. You will deliver recovery-oriented, trauma-informed interventions to adults in a mens Category C resettlement prison, supporting preparation for release.

Based on site, you will work as part of a multidisciplinary team, managing caseloads, completing assessments and care plans, and collaborating with partner services to promote through-care and successful resettlement.

Qualifications

• Commitment to recovery-focused, trauma-informed practice.
• Confidence working with individuals with substance misuse and low-level mental health needs.
• Willingness to complete mandatory 12-month Drug and Alcohol Workforce Development Programme.

Responsibilities

• Manage a caseload of service users.
• Complete assessments and care plans.
• Deliver structured 1:1 and group interventions.
• Provide harm reduction advice and guidance.
• Contribute to accredited treatment programmes where relevant.
• Collaborate with healthcare, probation, housing, employment and external drug services to support through-care and resettlement.
• Support service targets set by Forward and local commissioners.
