Job summary

The Forward Trust is seeking a Health and Wellbeing Practitioner in Cardiff to support male adults with substance misuse and low-level mental health needs. You will work one-to-one and in groups, building strong relationships with service users and partner agencies.

You will manage a caseload, assess needs, develop care plans and measure outcomes as part of a holistic, recovery-oriented approach. We welcome applicants from diverse backgrounds and offer a range of benefits.

Qualifications

• Experience in substance misuse, health and wellbeing, or mental health support settings.
• Excellent communication and interpersonal skills.
• Ability to build trusting professional relationships with service users.
• Ability to deliver both individual and group interventions.
• Able to assess needs and develop care plans with service users.

Responsibilities

• Manage a caseload of service users and provide ongoing support.
• Work with male adults experiencing substance misuse and low-level mental health needs.
• Provide one-to-one, face-to-face support.
• Deliver and facilitate group interventions.
• Complete assessments and develop care plans.
• Provide information and guidance on substance misuse and wellbeing.
• Refer to specialist services as needed.
• Collaborate with external agencies for coordinated support.
• Attend multi-agency meetings representing Forward Trust.
