Job summary

The Forward Trust is seeking a health and wellbeing practitioner for HMP Send, Surrey, 18.75 hours per week. You will support women with substance misuse and mental health challenges in a trauma-informed, gender-responsive setting.

Qualifications

• Experience supporting individuals with substance misuse, mental health, or complex needs.
• Excellent communication and relationship-building skills.
• A proactive, organised, and solution-focused approach.
• Confidence working within a multi-disciplinary environment.
• Strong IT and record-keeping skills.
• Commitment to completing the Drug and Alcohol Workforce Development Programme (DAWFDP).

Responsibilities

• Managing a caseload and coordinating individual support plans
• Conducting assessments and developing person-centred care plans
• Delivering structured one-to-one sessions and facilitating group programmes
• Providing harm reduction advice and guidance
• Supporting release planning and through-care arrangements
• Working closely with healthcare, mental health, and prison colleagues
• Maintaining accurate records and contributing to service performance targets
• Undertaking second signatory duties and oral swab testing where required
