Qualifications

Recovery Co-Ordinator

About the role

We are seeking an experienced full-time Recovery Coordinator to join our team, which includes GP Shared Care and Aftercare at the Primary Care Recovery Service (PCRS) in Lewisham.

You will manage a caseload of 50+ adult service users experiencing a range of substance misuse issues, especially alcohol and non-opiate. Your responsibilities will include devising, implementing, and reviewing ongoing recovery and treatment plans, and liaising closely with the entire service team and other agencies within Lewisham.

  • You will work with non-opiate and alcohol clients, completing triage assessments, one-to-one psychosocial key working sessions, Care Plans, Risk Assessments, TOPs, and supporting service users on their recovery journey.
  • You will assist service users in setting SMART goals and support them in achieving these goals safely and supportively. Recovery Coordination is a diverse and rewarding role.
  • You will liaise with other treatment providers, including healthcare professionals, social services for adults and children within Lewisham, and attend professional meetings, including weekly MDT team meetings.
Key Duties
  • Manage an adult caseload of non-opiate and alcohol clients.
  • Devise, implement, and review ongoing recovery and treatment plans.
  • Facilitate and promote individuals' progression from initial engagement through structured treatment to self-sustained recovery.
  • Reduce alcohol-related harm to individuals and the wider community.
  • Collaborate with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
