Job summary

Change Grow Live seeks a Recovery Coordinator for our Eastbourne team, covering Eastbourne, Uckfield and Crowborough hubs. The role is a fixed-term contract until 30 September 2027, full-time at 37.5 hours per week.

You will assess, engage and coordinate support for service users, helping them access right interventions at the right time. You will work with people affected by drug or alcohol use, building recovery plans, coordinating clinical and psychosocial support, and promoting harm

Qualifications

  • Experience in drug and alcohol services or related field.
  • Strong assessment, communication and case-management skills.
  • Able to balance independent work with a multidisciplinary team.

Responsibilities

  • Complete assessments, recovery plans, risk assessments and regular reviews.
  • Coordinate personalised treatment including clinical and psychosocial interventions.
  • Work closely with prescribing colleagues, health services and community partners.
  • Respond to safeguarding concerns and make appropriate referrals.
  • Promote harm reduction, naloxone, peer support and mutual aid.
  • Maintain accurate, timely and confidential records.

We have an exciting opportunity for a Recovery Coordinator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.


As a Recovery Coordinator within our Eastbourne team, you'll work alongside people affected by their own drug or alcohol use, as well as those concerned about someone they care about. This post is based in the Uckfield and Crowborough hubs.


You’ll help people understand their options, recognise their strengths and build a recovery plan around what they want to achieve.


This is not simply caseload management.


It is connection, coordination and meaningful change-combined.


Location:

Eastbourne, Uckfield & Crowborough


Contract:

Fixed Term Contract until 30th September 2027.


Hours:

Full time, 37.5 hours per week


Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and any associated payments will be pro rata based on contracted hours.


This role is subject to an enhanced Disclosure and Barring Service check.


Responsibilities

The impact you'll make

You’ll support people from their first contact with the service and throughout their treatment and recovery journey.


As their main care coordinator, you’ll complete assessments, understand their needs and bring together the right clinical, psychosocial and community support. You’ll help reduce harm, manage risk and connect people with services that can improve their health, wellbeing and independence.


You’ll:



  • Complete assessments, recovery plans, risk assessments and regular reviews.

What you’ll bring


  • You understand that effective support starts with listening.

  • You’ll be able to build trusted relationships, have honest conversations about risk and change, and remain compassionate when situations are complex.

  • You’ll bring experience from drug and alcohol services or a related field, alongside strong assessment, communication and case-management skills.

  • You’ll also be organised, confident using IT systems and able to balance independent working with being part of a supportive multidisciplinary team.

  • Most importantly, you’ll believe in people and their ability to make positive changes.
