Job summary

Change Grow Live in Crawley is seeking a Recovery Coordinator to assess and engage service users, coordinating clinical, psychosocial and community support to meet individual needs.

Based in the East team, you’ll travel across Crawley and Mid Sussex to support service users in their treatment and recovery journey. The role is permanent, 37.5 hours per week, full-time.

Qualifications

  • Strong understanding of substance misuse issues and experience in a similar field.
  • Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
  • Excellent verbal and written communication with good IT skills.
  • Proactive with strong time management and prioritising skills.
  • Good working knowledge of mental health interventions and services.
  • Ability to work in partnership with a wide range of professionals and stakeholders.
  • Ability to manage change prioritising service users’ needs.
  • Understand information governance processes and safeguards.

Responsibilities

  • Complete assessments, recovery plans, risk assessments and regular reviews.
  • Coordinate personalised treatment including clinical and psychosocial interventions.
  • Work with prescribing colleagues, health services and community partners.
  • Respond to safeguarding concerns and make referrals when needed.
  • Promote harm reduction, naloxone, peer support and mutual aid.
  • Maintain accurate, timely and confidential records.

We have an exciting opportunity for a Recovery Coordinator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

With this role you'll be working as part of our East team, based in our Crawley hub. As our East team covers the Crawley and Mid Sussex districts there will be some travel involved to support our service users where they are.

The team supports people with their drug and/or alcohol issues.

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

You’ll support people from their first contact with the service and throughout their treatment and recovery journey.

As their main care coordinator, you’ll complete assessments, understand their needs and bring together the right clinical, psychosocial and community support. You’ll help reduce harm, manage risk and connect people with services that can improve their health, wellbeing and independence.

You’ll:

  • Complete assessments, recovery plans, risk assessments and regular reviews.
  • Coordinate personalised treatment, including clinical and psychosocial interventions.
  • Work closely with prescribing colleagues, health services and community partners.
  • Respond to safeguarding concerns and make appropriate referrals.
  • Promote harm reduction, naloxone, peer support and mutual aid.
  • Maintain accurate, timely and confidential records.
About you
  • Have a strong understanding of substance misuse issues and experience of working within a similar field
  • Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions, services, and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
