Hours: Full Time, 37.5 per week
Contract Type: TBC 6-Months Fixed Term
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
With this role you'll be working as part of the Entry into Services (EIS) Team.
Assessment is the key opportunity to engage people and to ensure that they access the right intervention at the right time, in a way that best meets their needs.
The Entry into Services (EIS) Assessment worker will work with service users at the point of entry into the service.
The post holder will develop the service user plan (SUP) to record the ongoing assessment, review and management of goals, risks and actions.
You will advise service users about the options that are available to them both within the service and the wider community and offer them appropriate knowledge, advice and equipment to enable them to keep themselves safe. The team supports people with their or their loved ones' drug and/or alcohol issues.
