Job summary

The Forward Trust in Chelmsford is seeking a motivated RECONNECT Trainee to support individuals preparing for release from prison and reintegration into the community. You will work toward a Level 2 apprenticeship while gaining hands-on experience within the RECONNECT Team at HMP Chelmsford and surrounding Essex areas.

Qualifications

• Experience supporting vulnerable adults in a care or community setting.
• Willingness to learn and develop new skills.
• Capable of building positive professional relationships.

Responsibilities

• Processing referrals and providing updates to referrers after assessment.
• Learning to complete comprehensive needs assessments for referrals.
• Maintain regular contact with service users for up to six months after release.
• Support delivery of a holistic, safe service aligned with targets.
• Collaborate with colleagues and share best practice.
• Make referrals to the Meet and Greet service where appropriate.
• Help service users connect with local health, wellbeing and community services.
• Develop strong administration and case management skills including clear notes.
