The Forward Trust is recruiting a RECONNECT Support Worker to help people leaving prison access health and community services and to support their reintegration into the community in Bedfordshire. You will work with individuals before release and for up to six months after returning to the community.

We value lived experience and a compassionate, non-judgemental approach, with strong communication and organisational skills to manage a busy caseload and collaborate with partners across prisons

Qualifications

• Lived experience of the criminal justice system or related life experiences.
• Genuine empathy, non-judgemental approach and resilience.
• Excellent communication and listening skills.
• Ability to manage a busy caseload and work independently.
• Commitment to safeguarding and confidentiality.

Responsabilités

• Manage referrals and carry out comprehensive health and wellbeing assessments.
• Develop individualised care and support plans with each person.
• Build trusting relationships and provide ongoing support.
• Maintain accurate case notes, assessments and care plans.
• Support people to access GPs, healthcare providers and community services.
• Assist with preparation for release and reintegration into the community.
• Ensure safeguarding and risk management are applied at all times.
