An exciting post is now available for a Band 4 Therapy Assistant at Elmhurst- Discharge to Assess unit based in Winsford to work in a multidisciplinary bed based unit and support therapists in delivering high quality therapy interventions to patients.
Elmhurst is a 30 bedded unit providing round the clock nursing support to adult patients who are in transition from hospital to community.
This role offers the opportunity to enhance skills within a Discharge to Assess setting, linking in with the wider support network and being part of a integrated support system. You will work within Elmhurst to support patients rehabilitation and therapy needs.
It is expected that the location of this post will be within Elmhurst but with an assumption of supporting our colleagues across the CCICP footprint when needed.

We are looking for an enthusiastic, motivated and flexible Band 4 Therapy Assistant to join our friendly, supportive, dynamic team in Elmhurst.
The Therapy Assistant will take responsibility for the management of a defined workload of patients within Elmhurst following triage by the qualified therapist.
This will include independent assessment & formulation and progression of a person-centred rehabilitation plan. To work as part of the team providing a comprehensive rehabilitation service under the guidance of qualified staff for patients in the Discharge to Assess Unit.
There will be associated administrative duties and you will need to be computer literate with a good knowledge of Microsoft packages and have accurate keyboard skills.Our team utilises digital technology to support remote working and accommodate a flexible working approach where possible and is supported to do this with access to a laptop and mobile phone.
You are required to be a car driver and have access to your own vehicle.

Full induction on commencement of the role. Supervision from senior physiotherapist and Occupational Therapist. Setting individual objectives through appraisals and competencies will be provided. All generic therapy assistants benefit from regular supervision sessions which are fortnightly to start.
