Job summary

The Royal Devon University Healthcare NHS Foundation Trust is seeking a motivated physiotherapist to join the Community Therapy Team delivering patient-centred care in community settings across Devon. The role focuses on admission prevention, discharge support and rehabilitation for people with diverse disabilities and long-term conditions.

Working pattern is 24 hours per week on a 7-day rota, with interview date to be confirmed.

More detail about the role The Community Therapy Team work with people with a wide range of disabilities and acute or long term conditions striving to deliver excellent patient centred care. Our work is consistently varied and stimulating covering admission prevention and supporting discharge, and rehabilitation in a range of community settings through medical, social and public health approaches. The Community Therapy Team works within the broader Health and Social Care team alongside colleagues from Community Nursing and Adult Social Care. Working Pattern: 24hrs per week.
