Job summary

Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a Band 7 Patient Flow Manager to lead the Urgent Care Directorate's discharge and patient flow processes. The role focuses on safe, timely, and effective discharge planning in partnership with ward teams and community services.

You will coordinate multi-disciplinary teams, oversee Section 117 responsibilities where relevant, and drive continuous improvements in patient throughput to support our diverse community.

  • Planning discharge early by identifying patient needs, risks, barriers, and expected discharge dates.
  • Coordinating professionals such as doctors, nurses, social workers, occupational therapists, physiotherapists, community teams, housing, and voluntary services.
  • Ensuring safe aftercare by arranging follow-up appointments, medication, care packages, equipment, transport, and community support.
  • Managing complex discharges, including safeguarding concerns, homelessness, mental health needs, capacity issues, and delayed transfers of care.
  • Supporting communication with patients, families, carers, ward teams, and external agencies.
  • Reducing delays by escalating barriers, tracking discharge progress, and ensuring actions are completed.
  • Maintaining governance and documentation, including discharge plans, risk assessments, referrals, and escalation records.
  • Section 117 Aftercare responsibilities, helps ensure that discharge is clinically safe, legally compliant, and aligned with care planning requirements such as CPA, safeguarding, Mental Health Act considerations, section 117 aftercare where relevant, and effective liaison with community mental health services.

Working within the Urgent care Directorate as a patient flow manager, you will be responsible to ensure

  • Attending Ward Round
  • Liaising with other professionals including locality patient flow managers and Senior Urgent Care Nurse
  • Attending Section 117 meeting

You will need:

As a Patient Flow Manager, you will be responsible to coordinates safe, timely, and effective discharge from hospital or inpatient care back to the community or another care setting.
