Job summary

HCRG Care Group in Sittingbourne is seeking an Enabler Supervisor to lead a team of Enablers within the Intermediate Care and Reablement Service, delivering safe, personalised care to help people regain independence.

You will supervise, coach and develop staff, coordinate with therapists and social care colleagues, and ensure care plans stay focused on each service user’s goals. A full UK driving license is essential.

Qualifications

• NVQ Level 3 in Health & Social Care or equivalent.
• Experience working within community care, rehabilitation or intermediate care.
• Experience supervising and supporting care/clinical teams.
• Experience working as part of a multidisciplinary team.
• Understanding of Intermediate Care and rehabilitation pathways.

Responsibilities

• Supervise, support and develop a team of Enablers to deliver safe, effective and person-centered care
• Provide regular supervision, observed practice and spot checks
• Support the induction and development of new colleagues and help existing team members develop their skills and competencies
• Ensure individual care and reablement plans are followed appropriately and remain focused on each service user’s goals
• Promote independence by encouraging and supporting service users to regain skills and confidence in everyday activities
• Ensure care respects each person’s privacy, dignity, choices, cultural needs and communication preferences
• Build positive relationships with service users, their families and carers
• Undertake and contribute to risk assessments and ensure appropriate measures are in place to support safe care
• Recognise changes or deterioration in a service user’s condition and ensure concerns are escalated appropriately
• Work collaboratively with therapists, clinicians, social care colleagues and the wider multidisciplinary team
• Monitor standards of practice and help ensure learning from incidents, feedback and audits is embedded within the team
• Support investigations, safeguarding enquiries and quality improvement activity where required
• Work alongside administrative and operational colleagues to help coordinate staff and service-user requirements
• Respond effectively to staffing pressures and help ensure continuity of care when circumstances change
• Support the service’s on-call arrangements where required
• Maintain accurate and timely records and promote high standards of documentation across the team
• Promote a positive culture of learning, accountability, teamwork and continuous improvement
