Job summary

Perth and Kinross Council is seeking 6 Reablement Assistant posts based in Crieff to support the Health and Social Care Partnership. You will work in Home Assessment Recovery Teams (HART), visiting clients in their own homes to support independence, assist with daily living tasks, and help with medications under a structured schedule.

The role aims to prevent hospital admission and support safe, sustainable home living. Driving is essential; SVQ training is supported.

Qualifications

• Excellent listening, oral and written communication skills.
• Ability to maintain confidentiality and work with professionals, families and carers.
• Willingness to undertake SVQ in Social Services and Healthcare.

Responsibilities

• Visit and support people in their own homes following a pre-planned schedule.
• Promote reablement and independence while assisting with daily living tasks.
• Assist with personal care and meals, while maintaining dignity and safety.
• Administer and monitor medications and help with bedtime routines.
• Maintain records, complete risk assessments and report progress.
