Qualifications

• NVQ Level 2 in Health & Social Care (or equivalent) – Essential
• Experience in social care
• Experience administering medication using MAR charts
• Ideally experience in community care or reablement
• Full UK driving licence with access to a vehicle
• Business use motor insurance is essential
• Caring, reliable and professional
• Good communication and ability to work independently
• Passion for supporting independence

Responsibilities

• Providing high-quality reablement support to individuals in their own homes
• Supporting clients with everyday tasks to regain confidence and independence
• Following individual goal plans and helping achieve outcomes
• Assisting with personal care, daily routines and practical tasks
• Administering medication safely using MAR charts
• Monitoring and reporting changes in health or progress
• Maintaining accurate care records and documentation
• Supporting access to community resources and independence
• Working with families, carers and other professionals
• Promoting dignity, choice, wellbeing and person-centred care
