Job summary

Certitude in Lambeth is seeking a Reablement Support Worker to provide specialist support to Living Well Centre teams. You will help people in the community achieve person-centred outcomes and collaborate with social workers, therapists and other professionals to deliver best-practice support.

The role is 37.5 hours per week, permanent full-time, based in Lambeth. We offer training, development and a supportive, values-driven environment to help you progress your career in mental health support.

Qualifications

• Experience of supporting people with mental health needs and completing tasks with them.
• Experience of enabling people with mental health needs to take positive risks in their lives.
• Knowledge of safeguarding.
• A good working knowledge of health and social care services.

Responsibilities

• Provide intensive reablement support through practical hands-on work with and beside people to complete household chores.
• Offer rehabilitation support to help people re-learn domestic tasks from a strengths-based perspective.
• Assist with a range of reablement needs including domestic support, substance misuse and harm minimisation, physical health concerns, and money management.
• Build great relationships with the people we support and the people important in their lives

An exciting opportunity to join Certitude as a Reablement Support Worker at The Lambeth Living Well Network Alliance (LWNA).

Salary: £26,756 per annum

Hours: 37.5 hours

Job Type: Permanent – Full Time

Service: Mental Health

Location: Lambeth

Shift:Monday to Friday - 9am to 5pm

About the role

We are looking torecruit Reablement Support Workers to provide specialist expertise and support to Living Well Centre teams. They will facilitate the achievement of person-centred outcomes for people living in the community, some of whom have complex mental health and other support needs. Support workers will work collaboratively with professionals including care coordinators, social workers, occupational therapists, psychologists and psychiatrists as detailed in their support plans to achieve to achieve agreed outcomes and the provide the best practice support for clients.

The support worker will act as ‘social prescribers’ connecting people to local groups and services including peer support. They will also help people reconnect with family and friends.

Duties will include

• Provide intensive reablement support through practical hands-on work with and beside people to complete household chores such as shopping, cooking and cleaning.

• Offer rehabilitation support to help people re-learn domestic tasks from a strengths-based perspective
• Assist with a range of reablement needs which include domestic support, substance misuse and harm minimisation, physical health concerns, advice around Social Care and care packages, benefits and money management
• Build great relationships with the people we support and the people important in their lives
