Gateshead Council is seeking a Reablement Officer to support adults with physical and mental health needs to achieve personal goals and independence. The role involves hands-on case management, supervision of Enablement Workers, and collaborative working with health and social care partners.

The post offers a permanent part-time contract (22.25 hours) with flexi-time and hybrid working options, plus a comprehensive benefits package and ongoing training opportunities.

Qualifications

• Strong communication and interpersonal skills to support adults with care needs.
• Ability to work in multidisciplinary teams and coordinate with carers.
• Good IT literacy and ability to maintain accurate records and documentation.

Responsabilités

• Provide intensive enablement to maximise independence for adults with care needs.
• Carry out assessments and develop personalised support plans.
• Regularly review and adapt plans to changing needs.
• Manage a diverse caseload with families and carers.
• Lead Enablement Workers and Domiciliary staff with supervision.
