Job summary

Cardiff Council's Community Resource team Home Care (CRT) forms part of the Independent Living and Community Care Service and works with vulnerable adults to support them to live independently at home and stay connected to their communities.

This role involves line managing CRT Home Care staff, coordinating with health, social care and third sector partners, and developing pathways to support hospital discharge and prevent admissions.

Qualifications

• Experience leading staff in a regulated domiciliary care environment.
• Ability to extract, analyse and report care data to support service delivery.
• Experience collaborating with partner agencies in local government, health and third sector.

Responsibilities

• Support Senior Management Team to develop effective care pathways for discharge and reablement.
• Line manage CRT Home Care officers in line with statutory duties.
• Work with health, social care and third sector partners to strengthen partnerships and deliver corporate objectives.
• Promote preventative models focused on independence, outcomes and problem solving.
