Job summary

South Tyneside Council is seeking a Reablement Worker to join the Borrowdale House team, delivering therapy-led, person-centred reablement care after a hospital stay. This permanent role focuses on helping residents regain independence and rebuild confidence in daily living.

The post is 12 hours per week on a two-week rolling rota, with three 4-hour shifts per week. Shifts run 08:00–12:00, including weekdays, weekends and Bank Holidays, at Borrowdale House in South Tyneside.

Responsibilities

• Work closely with Therapy and Support Leads, Registered Manager, and other professionals to deliver high-level, person-centred care using a therapy-led, individualised reablement plan and risk assessments.
• Support daily routines and encourage clients to rebuild confidence in line with therapy guidance.
• Administer medication where required as part of care delivery.
• Collaborate with families and colleagues to ensure safe, person-centred care.
• Follow risk assessments and safeguarding procedures to protect service users.
