Job summary

Croydon Health Services NHS Trust is seeking a dynamic SPA Nurse (Band 6) to join our Single Point of Assessment and Rapid Response team. The role involves triaging community referrals, coordinating care, and supporting rapid response clinicians to assess patients promptly.

Qualifications

• Essential criteria include DN qualification (certificate, diploma or degree) or knowledge of community services.
• Registered nurse or paramedic registration.
• Recent triage experience.
• Ability to maintain access databases and Excel spreadsheets.

Responsibilities

• Coordinate and manage referrals for community services via the SPA.
• Triaging calls for the Rapid Response Service and support the Rapid Response clinician.
• Support timely assessments by the most appropriate clinician (nurse, OT or physio).
• Clinically triage all community health referrals and LIFE service referrals.
