Job summary

Gateshead Health NHS Foundation Trust is seeking a Band 6 Nurse Practitioner to join the Rapid Response Nursing Team in the community. This part-time role covers 30 hours per week across day, night, twilight, weekends and bank holidays, delivering high-quality, patient-centred care at the point of contact.

You will work within a multidisciplinary team to assess, plan, and deliver urgent and planned community care for adults, with a focus on safe, effective care and admission avoidance.

Qualifications

• Diploma or degree in Adult Nursing and current UK registration.
• Accredited clinical skills or equivalent expert knowledge.
• Teaching qualification or equivalent.
• Experience with PGDs and ability to work autonomously.
• Full valid driver’s license.

Responsibilities

• Assess, plan, implement, and evaluate nursing care in community settings.
• Deliver high-quality, evidence-based nursing care at patients' homes and in community environments.
• Undertake planned and unplanned clinical interventions independently.
• Provide timely assessment and treatment for complex health needs including frailty and palliative care.
• Administer treatments following clinical pathways, PGDs, and Trust policies.
• Promote patient safety and continuous improvement across the service.
• Support safe, 24/7 rapid response delivery and admission avoidance.
