Somerset NHS Foundation Trust is seeking a Therapist with broad general experience to work in unplanned care services within the new Somerset Intermediate Care Service. The role focuses on short-term interventions to prevent hospital admission and support rapid turnaround discharges.

You will work in a proactive, multidisciplinary team, follow standards and policies, and use enhanced assessment skills to manage patients identified for RR intervention, helping them stay at home and re-engage with

• Qualified professional in a therapy discipline.
• Experience in unplanned care is advantageous.
• Ability to work in a multidisciplinary team and follow standards.

Responsabilités

• Carry out specific therapy assessments to prevent hospital admission.
• Use enhanced skills to assess and manage patients identified for RR intervention.
• Collaborate with local neighbourhood teams to speed up discharges from A&E.
• Prioritise own caseload and delegate tasks to assistants.
