Job summary

Claire House is seeking an Associate Palliative Care Nurse Specialist (Band 6) to join the Rapid Response team across Claire House sites, hospitals, and the community. You will provide specialist palliative and end-of-life care for babies, children and young adults in settings of the family's choice—hospice, home or hospital.

You will participate in a 24/7 on-call service, work flexibly over a 7-day period and collaborate with a multidisciplinary team to develop care plans, supporting families

Qualifications

• Experience of complex care/palliative and/or end of life care.
• Perinatal/neonatal experience desirable.
• Excellent clinical skills and the ability to work in a small team.
• Ability to work autonomously and communicate effectively.
• Strong networking with internal and external professionals.
• Flexible and responsive to service needs.
• Able to drive and travel as required.

Responsibilities

• Provide specialist palliative and end-of-life care for babies, children and young adults in hospice, home or hospital settings.
• Work as part of the Rapid Response team across Claire House settings, local hospitals and the community.
• Participate in the 24/7 on-call service and work flexibly over a 7-day period.
• Collaborate with the multidisciplinary team to develop and implement care plans.
• Support families and offer counselling and guidance as appropriate.
• Maintain high clinical standards and further develop paediatric palliative care skills.
