Job summary

Buckinghamshire Healthcare NHS Trust seeks a Community Practitioner - Physiotherapist to join the Aylesbury team for maternity leave cover. The role involves providing expert community physiotherapy and managing a caseload to help people stay in their own homes and expedite discharge from hospital.

You will work within a multidisciplinary team including Physiotherapists, Occupational Therapists, Nurses and Paramedics, across Buckinghamshire.

Qualifications

• Provide expert Physiotherapy interventions and rehabilitation in the Community setting.
• Hold a community Physiotherapy caseload.
• Support patients to stay at home and enable early discharge.

Responsibilities

• Plan programmes of care to promote health and maximise independence.
• Active case management of patients through their journeys.
• Allocate workloads within the Rapid Response & Intermediate Care team.
• Collaborate with multidisciplinary colleagues to ensure timely care.
