Job summary

St Mungo's in West London is seeking an Assessment and Reconnection Worker to help people at risk of rough sleeping and support their pathway to a stable home. You will join a busy team delivering person-centred support, turning assessments into practical action plans and working on a weekly 24/7 rota, including weekends and bank holidays.

• Demonstrates ability to provide person-centred support to clients.
• Understands complex needs of people facing homelessness.
• Willingness to learn welfare benefit and housing guidance.

Responsibilities

• Be part of a busy team supporting people new to or at risk of rough sleeping.
• Provide personalised case management, turning assessments into action plans with client involvement.
• Build and maintain relationships to support clients through recovery.
• Develop knowledge of welfare benefit and housing legislation.
• Cover the weekly 24/7 rota, including weekends and bank holidays in mixed shifts.
