About the role

We are seeking an experienced Learning Disabilities Social Worker to join our Adult Social Care services. You will work with adults with learning disabilities to promote independence, protect their rights, and ensure their overall well-being. This is a rewarding opportunity to make a meaningful difference within a committed and supportive team

Key Responsibilities
  • Assessment & Care Planning - Carry out comprehensive assessments in line with statutory duties.
  • Develop and implement personalised, strengths‑based care plans that promote choice, independence, and positive outcomes.
  • Safeguarding - Identify, report, and manage safeguarding concerns.
  • Ensure all work adheres to local and national safeguarding procedures.
  • Case Management - Hold and manage a complex caseload, prioritising effectively and meeting service deadlines.
  • Maintain accurate, high‑quality case notes and documentation in line with professional and organisational standards.
  • Multi‑Agency Working - Work collaboratively with health partners, housing, education services, and voluntary organisations to deliver coordinated support.
  • Advocacy - Promote and protect the rights of adults with learning disabilities. Ensure individuals are actively involved in decisions affecting their lives.
  • Legal Compliance - Apply key legislation including the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
  • Professional Development - Participate in supervision, training, and ongoing professional development to maintain high standards of practice.
