Qualified Adult Social Worker, Haringey

Pay rate: £37 per hour

Vitalis is recruiting for a Qualified Social Worker to work in an Adults Team in Haringey. The role involves:

• Working within a strengths-based model of practice with a non-complex caseload to support residents in improving independence and wellbeing, collaborating with partners, and utilizing community resources.
• Undertaking strengths-based assessments, reviews, and support planning to maximize residents' independence, quality of life, and resilience.
• Providing practical support to residents and carers, such as assisting with living aids, welfare benefits advice, and arranging short-term care and respite services.
• Working with adult clients, relatives, carers, and communities to help them make informed decisions and contribute to support planning.
• Liaising with colleagues across the Haringey health and social care partnership for assessment and support activities.
• Maintaining accurate case notes, reports, and supporting legal processes.
• Reviewing care packages and contributing to multidisciplinary team meetings.
• Participating in safeguarding strategy meetings when required.
• Completing Continuing Health Care assessments.
