Job summary

Qualifications

• Experience in assessing needs and developing care plans.
• Ability to deliver 1:1 and group interventions.
• Ability to travel between projects if required.
• All probation-based roles require enhanced DBS checks.

Responsibilities

• Manage an allocated caseload, ensuring individuals receive appropriate and consistent support.
• Build positive, professional relationships with service users.
• Assess individual needs and develop care plans.
• Provide advice, guidance and practical support based on individual circumstances.
• Deliver structured 1:1 sessions and group work programmes.
• Support individuals to develop coping strategies and wellbeing.
• Collaborate with partners to provide joined-up support.
• Provide transitional support for individuals moving from prison into the community.
• Collaborate with health and wellbeing agencies to ensure continuity of care.
• Contribute towards service targets and outcomes.
• Accurately record and monitor outcomes.
• Develop and maintain relationships with partner organisations and professionals.
• Adapt approach to changing needs.
• Travel to other projects within the cluster when required.
• Work flexibly including evenings and weekends with notice.
