Job summary

Change Grow Live is recruiting a Recovery Coordinator to support service users from entry through their recovery journey, helping social reintegration and enabling meaningful lives. The role covers work across HMP Leyhill and HMP Ashfield within a dedicated restitution framework.

The post involves leading group interventions, coordinating with clinical teams, and ensuring effective release planning while upholding safeguarding and information governance. Not visa-sponsored.

Qualifications

• Strong understanding of substance misuse issues and prison/criminal justice context.
• Excellent verbal and written communication skills with good IT abilities.
• Proactive, able to work independently with strong time management and prioritisation.
• Good knowledge of mental health interventions and related services.
• Collaborative with diverse professionals and external partners.

Responsibilities

• Support service users from entry into the service through their treatment/recovery journey.
• Lead on preparation and delivery of Group Interventions.
• Collaborate with Clinical Substance Misuse team on care plans for clinical prescribing.
• Complete secondary screening assessment for prison arrivals as part of Integrated Health model.
• Reduce drug and alcohol-related harm among service users.
• Advocate for access to partnership services.
• Contribute to risk management processes, including ACCT when appropriate.
• Coordinate with Through the Gate, Healthcare, Prison partners and external agencies for Release Planning.
• Support social reintegration and promote recovery, resilience and self-determination.
