Job summary

Practice Plus Group's Primary Care Team at HMP Five Wells in Wellingborough, Northamptonshire, is recruiting a Primary Care Nurse to deliver nurse-led services across blocks, clinics and reception.

You will handle emergencies, chronic disease management and triage, supporting a wide range of healthcare services within a secure establishment. Full-time, 37.5 hours per week, shifts include days and nights, with a rolling 8-week rota.

Qualifications

• Registered nurse – Adult NMC with current registration.
• Evidence of Continued Professional Development (CPD).
• Understanding of multi-disciplinary and multi-agency working.
• Non-judgemental and compassionate approach with excellent communication.

Responsibilities

• Deliver a nurse-led service across blocks, triage rooms and healthcare departments.
• Undertake medication rounds, attend emergencies, and conduct reception screening.
• Provide nurse-led clinics and manage long-term conditions with the team.
• Deliver patient-centred care and act on feedback to improve services.
• Collaborate with partners to maintain safe, high-quality care.
