Job summary

Practice Plus Group in Lewes is seeking a Primary Care Nurse to join the Health in Justice service at HMP Lewes. You will work 37.5 hours per week on a rolling rota, including occasional nights and weekends, delivering nurse-led care within a multi-disciplinary team and supporting patients across the prison healthcare setting.

Qualifications

• Registered nurse – Adult.
• Current professional qualification (NMC).
• Evidence of Continued Professional Development.
• Understanding of multi-disciplinary and multi-agency working.

Responsibilities

• Deliver nurse-led care within a multi-disciplinary team to prevent hospital admissions.
• Undertake medication rounds and manage controlled medications.
• Run nurse-led clinics for your patient group with MDT support.
• Provide evidence-based, personalised care informed by patient feedback.
