Job summary

Qualifications

• Make decisions on suitability of new referrals following agreed protocols.
• Refer unsuitable clients to the relevant service or back to the referral agent as necessary.
• Assess and support people with mental health problems in self-management of recovery.
• Undertake person-centred interviews to identify change and assess risk to self/others.

Responsibilities

• Provide Low intensity interventions (e.g., psycho-educational interventions, Guided Self Help) drawing from CBT theories.
• Implement and support evidence-based interventions using a self-management model.
• Attend multidisciplinary meetings relating to referrals or clients in treatment.
• Maintain high standards of clinical record keeping and data entry on agreed systems.
