Job summary

The Forward Trust is hiring a Health and Wellbeing Practitioner to support people in prison across HMP Usk and HMP Prescoed. You will manage your own caseload, deliver harm reduction advice, and develop person-centred care plans to improve health and wellbeing and prepare individuals for life after release.

The role requires excellent communication, ability to work with diverse service users, and experience delivering 1:1 and group interventions.

Responsibilities

• Manage your own caseload, building positive and supportive relationships with service users.
• Provide appropriate harm reduction advice, guidance and support.
• Complete comprehensive assessments and formulate individualised care plans.
• Work collaboratively with service users to ensure co-production and that goals/needs remain at the heart of care.
• Deliver structured 1:1 interventions and support sessions.
• Facilitate engaging and effective group-work sessions.
• Support individuals with release planning and transition back into the community.
• Coordinate through-care and transitional support, ensuring connection with services on return to the community.
• Maintain accurate, timely and high-quality records with data capture.
• Work towards targets and demonstrate impact through data.
