Job summary

Hywel Dda University Health Board is recruiting for Local Primary Mental Health Support Services staff to deliver high-quality care within a multidisciplinary team across community settings in Wales.

The role focuses on governance, risk assessment, patient-centred interviews and delivering evidence-based psychological treatments (eg CBT) by face-to-face or remote methods, with participation in MDT meetings and a structured job plan.

Responsibilities

• Governance measures to support safe and effective practice.
• Decide on suitability of new referrals; signpost/escalate as needed.
• Conduct patient-centred interviews with risk assessment and safety planning.
• Provide information on evidence-based high-volume psychological treatments (eg CBT) via face-to-face, group, telephone or other media.
• Attend multidisciplinary meetings related to referrals or clients in treatment.
• Adhere to an agreed job plan and manage workload within policies.
