We have an exciting opportunity for a Primary Care Transformation Manager to join our team at Rosedean House Surgery on a fixed-term 6 months contract.

We’re looking for an exceptional Primary Care Transformation Manager to lead and deliver strategic improvements across our GP practice. This is more than a management role; it’s a unique opportunity to shape the future of healthcare delivery in your community.

About the Role

As our Primary Care Transformation Manager, you’ll be at the heart of change, leading key initiatives that modernise general practice, improve patient care, and align with NHS and OneMedicare priorities. You’ll work closely with practice teams, Primary Care Networks (PCNs), Integrated Care Boards (ICBs), and other stakeholders to ensure seamless, high-quality service delivery that meets the needs of the local population.
Your leadership will help embed efficient systems, support income generation, ensure compliance, and most importantly, make a meaningful impact on the lives of our patients.

What You’ll Be Doing

  • Deliver transformation projects that improve access, efficiency, and patient outcomes.
  • Translate strategic goals into real-world improvements at practice level.
  • Drive innovation and implement modern general practice initiatives.
  • Support CQC readiness through audits and training.
  • Work with clinical and non-clinical staff to embed sustainable change and continuous improvement.
  • Monitor and improve performance on QoF, IIF, LES/DES, and other NHS income streams.
  • Champion excellent patient engagement and experience, growing the practice list size.
  • Build strong stakeholder relationships and promote OneMedicare locally.
  • Provide day-to-day leadership, coaching, and development for practice teams.

  • Proven experience in Primary Care management and transformation.
  • In-depth knowledge of GMS contracts, NHS structures, and Primary Care operations.
  • Strong leadership skills with a passion for developing people and services.
  • Ability to manage complex change while maintaining operational excellence.
