The Practice Plus Group delivers nurse-led healthcare within HMP Five Wells, a category C male prison. You will run nurse-led clinics, support a multidisciplinary team, and manage medication rounds across seven days on a rolling rota. The role requires NMC registration, vetting, and ongoing professional development.

Responsibilities

• Deliver nurse-led clinics to a patient group supported by a multidisciplinary team
• Deliver a nurse-led service and prevent hospital admissions
• Undertake medication rounds and dispense controlled medication
• Attend emergency response situations
• Assess patients and undertake reception screening
• Provide cover for reception
• Work across house blocks, triage rooms, wings and the healthcare department
• Support healthcare services including emergency care, clinics and long-term condition management
• Provide evidence-based, personalised care informed by patient feedback
• Deliver non-judgemental care
• Build links with partners and stakeholders
• Promote best practice, clinical supervision and continuous quality improvement

Job description

This post is in the Primary Care Team at HMP Five Wells, a category C male prison run by the Health in Justice service of Practice Plus Group. The nurse delivers nurse-led clinics, medication rounds, triage, reception screening and emergency response, including management of long-term conditions. The role runs seven days a week on a rolling eight-week rota, with weekday, weekend and night shifts within a 37.5-hour week. Adult NMC registration is required, and offers are subject to vetting and security checks.
