We're looking for a Primary Care Nurse who is skilled, passionate about community health, enjoys building connections with patients, and wants to join a team that feels more like a family.
Ideally, you will have diabetic experience and be responsible for managing diabetes, including insulin initiation, health education, blood review, medication monitoring, and dose adjustment. Training in this area may be provided for the right candidate.
Working as part of the nursing team, delivering care within your scope of practice to the patient population. The role includes supporting clinical policy review and development.
You will deliver clinic and home-based treatment plans, including education, lifestyle advice, symptom and medication management.
The service aims to provide access to diagnostics and early treatment. You will work closely with secondary care partners to ensure seamless patient transitions and build relationships with external organizations to promote joined-up care.
