Job description

We're on the lookout for a Primary Care Nurse who is skilled, passionate about community health, enjoys building connections with patients, and wants to join a team that feels more like a family.

Ideally, you will have diabetic experience and be responsible for managing diabetes, including insulin initiation, health education, blood review, medication monitoring, and dose adjustment. Training in this area may be considered for the right candidate.

Working as part of the nursing team, delivering care within your scope of practice to the entitled patient population. The role includes supporting the management team in reviewing clinical policies and procedures.

The post holder will deliver clinic and home-based treatment plans, including education, lifestyle advice, and the monitoring and management of symptoms and medications.

The service aims to provide access to diagnostics and early treatment intervention. The post is integral to the multidisciplinary team, working closely with secondary care partners to ensure seamless patient transitions to primary care, underpinned by solid clinical governance. The post holder will build relationships with external organizations to promote joined-up patient care and services.

Main duties of the job

As a Practice Nurse focusing on Chronic Disease Management, you will support patients in living well with long-term conditions. You will work autonomously and collaboratively to deliver evidence-based care, education, and support.

Key Responsibilities:

  • Manage and monitor patients with chronic diseases such as Diabetes, Asthma, COPD, and Cardiovascular Disease.
  • Conduct health assessments, physical examinations, and investigations.
  • Develop, implement, and evaluate individual care plans with patients and the multidisciplinary team.
  • Provide patient education and self-management advice.
  • Monitor clinical data and adjust care plans accordingly.
  • Administer medications following protocols.
  • Participate in multidisciplinary team meetings and contribute to service development.
  • Stay updated with current practices and guidelines.
  • Perform general practice nursing duties such as immunizations, wound care, and health promotion.

Job responsibilities

Clinical duties include, but are not limited to:

  • Managing chronic diseases including Diabetes, COPD, Asthma, and CHD/CVD.
  • Performing cervical screening and promoting breast awareness.
  • Administering adult and pediatric injections.
  • Providing immunizations and participating in annual appraisals.
  • Engaging in clinical and practice meetings and developing clinical protocols.
  • Ensuring safe storage and disposal of vaccines and drugs.
  • Undertaking venipuncture and ECGs.
  • Offering lifestyle advice and supporting behavior change.
  • Participating in smoking cessation and travel health services.
  • Recognizing safeguarding issues and working with relevant teams.
  • Applying infection control measures and managing emergencies.
  • Providing family planning services and auditing care outcomes.
  • Maintaining disease registers.
Person Specification
Experience
  • Primary care experience and chronic disease management.
  • Evidence of nurse revalidation and CPD.
  • Strong organizational and problem-solving skills.
  • Experience in infection control, cold chain management, audit, and risk management.
  • Driver with own car.
  • Experience in community or primary care settings.
Qualifications
  • Nursing degree or equivalent, with primary care registration.
  • Qualified in managing Diabetes, Asthma, COPD, and CVD.
