Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective value‑based primary care to adults on Medicare in outpatient settings. You will practice independently within a collaborative care team, focusing on population health and preventive care to keep patients healthy and out of the hospital.

Requirements include a Master of Science in Nursing or DNP, national certification (FNP/Adult-Gerontology), active NP license and US work authorization.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care NP, etc.
• Active NP license and US work authorization.

Responsibilities
