South West London and St Georges Mental Health NHS Trust is recruiting for a Primary Care Mental Health Practitioner in Wandsworth PCN. The role provides supervised, brief psychological interventions to GP patients, with collaboration across GP practices and mental health services.

It involves assessments, person-centred planning and potential onward referrals, with some remote working opportunities. The post offers development pathways and CPD in clinical psychology areas within the Trust.

Formación

• Relevant clinical experience
• At least twelve months experience with people who have experienced serious ongoing mental health problems
• Experience of supervision and/or mentoring of junior staff and students
• Upper second class honours degree or higher in psychology
• Entitlement to graduate membership of the British Psychological Society (BPS) and CPD

Responsabilidades

• Provide supervised direct clinical contact with clients in GP practices.
• Attend weekly meetings and liaise with GP practices and other mental health services.
• Complete assessments and collaboratively develop person-centred plans.
• Offer brief psychologically informed interventions and onward referrals as needed.
• Support transition from primary to secondary care when more specialist support is required.
• Promote safety and wellbeing of service users and their families.
• Develop close relationships with Primary Care practices and participate in practice meetings.
