• Provide supervised, direct clinical contact with clients in GP practices.
• Develop person-centred plans after assessments.
• Liaise with GPs and other professionals within the mental health pathway.

Responsabilidades

• Complete assessments and collaboratively develop person-centred plans.
• Offer follow-up sessions to provide brief psychologically informed interventions.
• Support clients and navigate referrals to appropriate services (secondary care where needed).
• Work with clients and team to promote engagement with services and self-management.
