NSFT is recruiting a Band 6 Mental Health Practitioner to join the NN4 Primary Care Network across GP practices in North Norfolk. You will work embedded within GP teams, delivering non‑urgent mental health assessments and brief interventions for adults 16+.

The role is jointly funded under ARRS, supporting Place‑Based Care and timely access in familiar GP settings. You will collaborate with multidisciplinary colleagues, provide short‑term support, psychoeducation and safety planning, and

Qualificações

• Registered Mental Health or Learning Disability Nurse, Occupational Therapist or Social Worker with the relevant professional registration.
• Practice Assessor and Practice Supervisor training (NMC Registrants) or Practice Educator (AHP Social Worker Registrants).
• Ongoing Professional Development working towards degree level (if professional registration is following a diploma level course).

Responsabilidades

• Provide assessments, brief interventions and short‑term support for people aged 16+
• Support individuals with presentations including anxiety, depression, trauma-related distress, bipolar disorder, psychosis and other mental health needs
• Deliver psychoeducation, problem‑solving approaches, safety planning and self‑management strategies
• Work collaboratively with GP practice staff and local services to ensure people are directed to the most appropriate support
• Promote recovery, wellbeing and prevention of deterioration within local communities
